Wart Care Problems We Correct
- Non-warts treated as warts — pearly papules and Fordyce spots needlessly "treated" elsewhere; examination ends the confusion.
- Molluscum mistaken for warts — a different virus needing different removal; we distinguish them on examination.
- Pharmacy self-treatment burns — aggressive acids scar genital skin permanently.
- Secondary syphilis mimics (condylomata lata) — flat moist plaques that look wart-like; we screen serology when the picture suggests it — missing this means missing syphilis.
Complete Care for a Common Problem
Genital warts are the visible signature of low-risk HPV (types 6 and 11 cause ~90% of them) — benign growths that are more psychologically distressing than medically dangerous. Yet the care around them is often poor: misdiagnosis of normal anatomy as warts, removal without explanation, no recurrence counseling, and no discussion of the partner or the vaccine.
A dedicated wart clinic covers the full arc: examination first (because papules, Fordyce spots, molluscum and skin tags mimic warts), removal matched to the lesions, recurrence management as the virus clears, and prevention — including the honest role of condoms and the real role of Gardasil-9.
This page describes complete wart care at our Kathmandu clinic — what happens at each stage and what it costs.
First: Is It Actually a Wart?
Before any treatment, examination answers the real question. Genital warts are typically soft, flesh-colored, cauliflower-textured growths on the genitals, perianal area or groin — but the mimic list is long and matters:
Pearly penile papules
Uniform dome bumps ringing the glans — normal anatomy, zero treatment needed.
Fordyce spots
Small pale sebaceous glands on the shaft or labia — normal, harmless.
Molluscum contagiosum
Smooth bumps with a central dimple — different virus, different treatment.
Skin tags
Soft pedunculated growths — benign, removed only for comfort.
Condylomata lata
Flat moist syphilis lesions — the dangerous mimic; serology distinguishes them.
Precancerous lesions (VIN/PeIN)
Pigmented or fixed lesions — biopsied, never just ablated.
Removal Options, Matched to You
No single method suits every case — lesion number, size, site and your preference determine the choice:
- Cryotherapy — liquid nitrogen freezing; the standard for most presentations, 1–2 weekly sessions.
- RF cautery — one-session removal for larger or clustered lesions under local anesthetic.
- TCA — clinician-applied acid for moist-area warts; the pregnancy-compatible option.
- Imiquimod / podophyllotoxin — prescription home treatment for suitable cases.
- Combination — procedure plus topical maintenance reduces recurrence in stubborn cases.
The Partner Question
Partners of wart patients usually ask two questions — "should I be checked?" (reasonable — examination is quick) and "who gave it to whom?" (unknowable — HPV can persist silently for years before showing). We handle both conversations factually and without blame. Partners are offered examination and, importantly, vaccination assessment — since protecting against the HPV types they have not yet encountered is the genuinely useful move.
The Long-Term Picture
- Recurrence is expected biology — ~20–30% early on, declining as immunity controls the virus.
- Most patients clear completely — 6–12 months without recurrence usually means lasting clearance.
- Immune review when stubborn — persistent aggressive recurrence triggers health evaluation including HIV screening.
- Vaccination alongside — Gardasil-9 covers types you haven't met, including the cancer-risk types.
- Smoking matters — smokers clear HPV more slowly; quitting is the real "immune booster."

Frequently Asked Questions
Are genital warts dangerous?
The wart types (HPV 6/11) are low-risk — they do not cause cancer. They are contagious and distressing but medically benign; the dangerous HPV types are invisible and handled via screening.
How are warts different from normal bumps?
Texture and pattern — warts are typically irregular and cauliflower-like; papules and Fordyce spots are uniform and symmetric. A short examination distinguishes them reliably.
Do warts mean I have HIV?
Usually not — but persistent aggressive wart recurrence can signal immune suppression, which is why we may suggest HIV screening in stubborn cases. Routine warts alone do not imply HIV.
Can I treat warts at home?
Only with the right prescription — imiquimod or podophyllotoxin for suitable cases. Pharmacy hand-wart products will burn genital skin; do not use them.
Is wart removal permanent?
Removal clears the lesion; the virus clears over months. Early recurrence is possible — we plan for it honestly and retreat as needed until immunity finishes the job.
Can warts be inside?
Yes — vaginal, cervical, urethral and anal-internal warts occur. Internal evaluation is part of a complete assessment when indicated.
