HPV in Nepal — the Practical Picture

  • Cervical cancer is among the leading cancers in Nepali women — almost entirely preventable with screening and vaccination.
  • HPV vaccination coverage remains low — many adults were never offered it; catch-up vaccination still benefits adults up to 45.
  • Warts are treated casually — pharmacy creams and unqualified "removal" lead to scarring and recurrence; proper ablation is quick and safe.
  • Anxiety exceeds the science — most HPV clears itself; our counseling separates the real risks from the fear.

HPV: One Virus, Several Different Problems

Human papillomavirus is not one disease — it is a family of 200+ types split into two clinical stories. Low-risk types (6, 11) cause genital warts: visible, treatable, never dangerous. High-risk types (16, 18 and others) are silent, clear spontaneously in most people within 1–2 years, but — when they persist — drive nearly all cervical cancer, much anal and penile cancer, and a rising share of throat cancers.

Good HPV care means knowing which problem you are managing: warts need treatment, high-risk infection needs surveillance, and the whole thing is largely preventable with the HPV vaccine — one of the few vaccines that prevents cancer.

At Thamel Clinic in Kathmandu we provide the complete HPV service under one roof: DNA testing and cervical screening coordination, same-visit wart removal, Gardasil-9 vaccination for eligible patients, and honest counseling about a virus nearly every sexually active adult acquires at some point.

The Complete HPV Service Menu

HPV DNA Testing

Molecular testing identifies high-risk types — usually paired with cervical screening in women over ~30; also available for men where clinically indicated.

Cervical Screening

Pap smear / liquid-based cytology and HPV co-testing — the program that prevents cervical cancer. Colposcopy referral coordinated when abnormal.

Gardasil-9 Vaccination

Nine-valent vaccine covering the types behind ~90% of warts and most HPV cancers — for teens and adults up to 45 after assessment.

Genital Wart Treatment

Cryotherapy, RF cautery and prescription topicals (imiquimod, podophyllotoxin) — same-visit removal for most lesions.

Anal/Penile Evaluation

High-risk-site examination and referral pathways where indicated, especially for MSM and immunocompromised patients.

Counseling & Partner Guidance

What a positive HPV result actually means, what it does not mean, and what partners need to know (usually: nothing to do).

HPV Testing: What It Is and Isn't

HPV DNA testing detects high-risk types from a cervical sample — its main role is cervical cancer prevention, typically combined with Pap cytology in women 30+. A positive high-risk result does not mean cancer; it means surveillance intensity increases, because persistent infection — not transient infection — is what matters.

There is no approved general "HPV test" that predicts warts or screens men. Diagnosis of warts is clinical — visual examination by the doctor. That is an important honesty point we make to every patient asking for a "whole-body HPV test."

Gardasil-9: The Cancer-Prevention Vaccine

Gardasil-9 covers nine HPV types — the seven high-risk types behind ~90% of cervical cancers plus types 6 and 11 behind ~90% of genital warts. It works best before exposure (routine age 9–14, two doses) but remains beneficial for adults through age 45 — you are unlikely to have encountered all nine types, and the vaccine still protects against the ones you have not met.

Three doses over six months for adults. It does not treat existing infection — but it prevents the types you have yet to encounter, which is meaningful protection at any age under 45.

Genital Wart Treatment

Visible warts are removed in-clinic: cryotherapy (liquid nitrogen freezing), radiofrequency cautery for larger lesions, or prescription topicals (imiquimod immune cream, podophyllotoxin) for suitable home treatment. Removal is quick and done under local anesthetic where needed. Honest counseling: the virus may persist after the wart is gone, so recurrence (~20–30% within months) is possible — and usually diminishes as your immune system suppresses the virus.

Frequently Asked Questions

Is HPV curable?

The virus itself is not treated — but ~90% of infections are cleared by your immune system within 1–2 years. What we treat are the effects: warts are removed, abnormal cervical cells are caught and managed. Prevention via vaccine is the real power move.

Does a positive HPV test mean I'll get cancer?

No — most HPV clears spontaneously. A high-risk positive means closer surveillance (more frequent screening, colposcopy if indicated) so any changes are caught years before they could become cancer.

Should I vaccinate if I'm already sexually active?

Usually yes, up to 45 — you will not have met all nine covered types. The vaccine cannot clear existing infection but protects against the types you have not encountered.

Can men be tested or vaccinated?

Vaccinated — yes, and recommended. Routine HPV DNA testing is not standardized for men; wart diagnosis is by examination. MSM and immunocompromised men get site-specific evaluation where indicated.

My partner has warts — what should I do?

Come in for examination — you may already carry the virus (it is near-ubiquitous). We assess, discuss vaccination to cover remaining types, and explain transmission honestly: it is common and usually harmless.

How often should I do cervical screening?

Standard guidance: Pap every 3 years from ~21–29, and Pap + HPV co-testing every 5 years (or HPV primary screening) from 30–65 — individualized to your history at consultation.
