Confidential HIV Testing

Best HIV Testing Center in Kathmandu 4th Generation & RNA Testing, Done Right

HIV testing is only as good as the generation of the test and the honesty of the counseling around it. Thamel Clinic offers 4th-generation Ag/Ab testing, RNA PCR for early detection, and clear window-period guidance — all fully confidential.

HIV Testing in Nepal — Practical Facts

  • Nepal's national program provides free ART through government centers (NAST network) — a positive result here connects you to that system plus our private support.
  • 2nd/3rd generation rapid tests are still common at cheap labs — they miss early infection that 4th-gen would catch.
  • Fear of the test exceeds fear of the virus — delayed testing is the biggest risk factor we see; a 15-minute visit ends months of anxiety.
  • PEP is time-critical — if your exposure was within 72 hours, do not wait for window-period math: come in now, PEP may apply.

HIV Testing: Method, Window, Meaning

Three decisions determine whether an HIV test result can be trusted: which generation of test, when it is taken relative to exposure, and who interprets it. A 4th-generation laboratory Ag/Ab combination detects both HIV antibodies and the p24 antigen — reliable from about 18–45 days post-exposure. HIV RNA PCR detects the virus itself and can show infection from roughly 10–33 days — the earliest reliable option.

A good testing center does not just run the test — it tells you which test your timeline needs, gives pre-test counseling that answers the questions you were afraid to ask out loud, delivers results with post-test counseling either way, and connects positive results to care immediately rather than abandoning you with a printout.

This page explains the test types available in Kathmandu, the real window periods, and exactly how testing works at our clinic — including how your privacy is protected end to end.

HIV Test Types Explained

4th Generation Ag/Ab (Standard)

Laboratory test detecting both p24 antigen and antibodies — reliable from ~18–45 days post-exposure and conclusive per guidelines at 45 days. Our default diagnostic test.

HIV RNA PCR (Earliest)

Detects viral RNA directly — positive from ~10–33 days post-exposure. Used for very early detection, window-period resolution, and confirming ambiguous results.

Rapid/self tests (2nd–3rd gen)

Finger-prick antibody tests — convenient but only reliable after ~90 days. A negative early rapid test is not conclusive.

Confirmatory testing

Any reactive screening result is confirmed per the national algorithm (second test / differentiation assay) before an HIV diagnosis is ever declared.

Window Periods Without the Myths

The "window period" is the gap between infection and a test's ability to detect it. It differs by test — and internet fear-mongering wildly exaggerates it:

  • HIV RNA PCR: ~10–33 days. The earliest reliable detection available.
  • 4th gen Ag/Ab (lab): ~18–45 days. Conclusive at 45 days per current CDC guidance.
  • Rapid antibody tests: up to ~90 days to be conclusive.
  • The 3-month/6-month fear is outdated for 4th-generation testing — 45 days is the modern conclusive mark for lab Ag/Ab.
  • Within 72 hours of exposure: skip the test-timing debate entirely — PEP assessment comes first.

How Testing Works at Thamel Clinic

  • Private pre-test counseling — your exposure, timing, and the right test for it; your questions answered first.
  • Single blood draw — 4th gen lab test and/or RNA PCR depending on timeline.
  • Results in 24–48 hours — delivered to you alone, by the channel you choose.
  • Post-test counseling either way — negative results come with window-period clarity and prevention advice (including PrEP discussion).
  • Positive results connect immediately — confirmatory testing, baseline labs, ART linkage and ongoing doctor support — never just a report.

Who Should Test

  • Everyone, at least once — global guidelines recommend every adult be tested at least once in life.
  • After any unprotected sex with a partner of unknown status — once the window closes.
  • Every 3–12 months for MSM, sex workers, people who inject drugs, and anyone with multiple partners.
  • After needle-stick or blood exposure — occupational or otherwise.
  • Before marriage or pregnancy — standard, sensible, confidential.
  • Alongside any other STI diagnosis — a positive gonorrhea or syphilis result always prompts HIV testing.

Frequently Asked Questions

Is a 4th generation test conclusive at 45 days?

Per current CDC guidance, a laboratory 4th-gen Ag/Ab test is conclusive at 45 days post-exposure — the old 3–6 month advice applied to older antibody-only tests.

I was exposed yesterday. Should I test?

Come in now — but for PEP assessment, not testing. Post-exposure prophylaxis must start within 72 hours. Testing schedules follow afterward.

What does a reactive result mean?

It is a screening flag, not a diagnosis — every reactive result goes through confirmatory testing before any conclusion. False positives on single rapid tests do happen.

Will anyone be told my result?

No — results are released only to you, through your chosen channel. Nepal's HIV care system is equally confidential if linkage is needed.

Can I test anonymously?

We can arrange essentially anonymous testing — minimal identifying details, results delivered privately. Tell us your preference at intake.

If positive, is treatment really free in Nepal?

Yes — antiretroviral therapy is provided free through the national program. We manage the confirmatory work-up, linkage, and ongoing private support around it.
